Visiting the Surgery to Register

Highfield Surgery welcomes patients for registration regardless of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation.

We accept new patients who reside in the Hemel Hempstead area. If you wish to register with us, please collect a registration form and a new patient Questionnaire from reception.

We offer all our new patients a New patient check with our nurse, we would advise you to have this if you have any medical conditions.

If you are on medication please make an appointment with a doctor  before your current medication runs out as we will not be able to issue any medications before you are seen.

If, as a patient, you move out of our practice area it may be necessary for you to register with a practice nearer to your home.
